Who needs a pre-op exam?

How do I know if I qualify for a Pre-Op at ExpressCare?

Below includes detailed information on patients who may not qualify for a pre-op at ExpressCare

Patients NOT Eligible for Pre-Op Clearance at ExpressCare (ASA Level III)

For patient safety, individuals with the following conditions should be referred to a higher level of care for preops like their primary care provider or our team may refer you to LifeBridge Health’s Sinai Hospital POWR Clinic.

Substance Use Disorders

  • Any substance abuse disorder (excluding tobacco or marijuana use)
  • Includes alcohol use disorder and drug use disorders

Severe Obesity

  • Body Mass Index (BMI) of 40 or higher

Uncontrolled High Blood Pressure

  • Blood pressure over 140/90 despite medication use

Uncontrolled Diabetes

  • Blood glucose consistently above 180 despite treatment
  • May include symptoms such as:
    • Frequent urination
    • Excessive thirst
    • Extreme fatigue
    • Unexplained weight loss
    • Blurred vision
    • Slow-healing wounds

Uncontrolled COPD

  • Recent worsening requiring additional medications
  • Two or more flare-ups in the past year
  • Using a rescue inhaler more than twice per week

Severe Asthma

  • Requires medium- or high-dose inhaled corticosteroids plus additional long-acting medications

Significant Cardiac Conditions

  • Any condition requiring ongoing cardiology follow-up
    (Examples: heart failure, prior heart attack, pacemaker/defibrillator)
  • These patients require cardiac clearance

Recent or Complicated Stroke (CVA)

  • Stroke within the past 6 months
  • Or any stroke with ongoing neurological deficits

Kidney Failure

  • End-stage renal disease (ESRD)
  • Any patient on dialysis

Liver Disease

  • Active hepatitis
  • Advanced liver disease (fibrosis or cirrhosis)
